PROGRAM TRAINING SESSION - PAYMENT REQUEST FORM

Coaches: Submit this form within 24 hours after every Program Training Session you support that is not part of your fixed contract requirements.

Includes: Signature Trainings, Special Programs, and other ad hoc training program requests.

Does NOT include: Contracted team trainings, contracted age-group tryouts, team tryouts, camps (tracked separately), or coach game coverage (handled directly between coaches and not submitted through this form).

Approval & Payment: All submissions are verified and approved by your Coaching Director. Approved Program Training Sessions are submitted for payment on the 1st of the following month (for the prior month). Payments are processed separately from your fixed monthly contract payments and are deposited within the first 3 business days of the month.

Late submissions: require special DOC approval and will be paid in the following pay cycle.
