Uniform Fee Agreement *

I understand that uniforms are not included in my registration fees and agree to purchase a uniform ($75-$200-ish) once teams are formed and team colors are determined

Uniform Deadline Agreement & Authorization *

I authorize Elite Soccer League to purchase a uniform for my child (using the sizing information provided above) and to charge my credit card for the uniform ($75-$200-ish) if I have not done so by August 15

Seasonal Play Agreement *

I understand that the majority of youth soccer costs are associated with field rental, field/player insurance, and training/admin expense. If we are unable to play games for any reason (e.g. global pandemics, natural disasters, etc.), I understand that the league will organize training sessions and these sessions will constitute the fall season. As long the league provides training opportunities I will not require a refund, even if actual games are not able to be played. Also, I understand that the Signature program offers paid training options for coaches who request assistance. If my coach opts to have a paid trainer support the team, I authorize an additional charge of $150-$225 to be added to my credit card on file for three months of training ($50-$75 per month, depending on if the trainer takes trainings only or training plus games)
